• Beneficiary / Survivor Pensioner (SD) 40 years old and above in good health condition
• GD/ITF pensioner 6 months term and below in good health condition
• SP/ST pensioner 6 months term and below still in good health condition

• Clear copy of client’s ATM (Front Only)
